Application for Assistance

To apply, simply print the Application for Assistance and complete the application in full and return it to our main office. 

• All applications must be returned in person.

• Applications will not be accepted by mail.
•  
• Our waiting list is organized by Preference Point first, then date and time of application.
•  
• We do not provide waiting list location or rank.  We are unable to provide applicants with timeframes of when assistance will be received.  When assistance is available, applicants will be notified in writing.
